Fill in this form to register for fall 2025 

All forms must be accompanied by a $60 registration fee 

Registration is for the 2025-26 academic year with Dara Fensky at the following rates:

$60/60 min  $45/45min lessons for my first child,
and
$52/ 60 min $39/45 minutes for my second child

 

Lesson year will be from September 7th to June 20th, 34 lessons will be offered

Two or more performance opportunities will be available in Crossfield and area

Lessons will be scheduled at a fixed spot for the lesson year. This is your spot and will not be giving to any one else during the lesson year unless lessons are terminated. 

In event you can not make your lesson, online lessons are available and select rescheduling is offered depending on availability.

Lesson Policy agreement:

I understand lesson fees will be invoiced at the end of each month for the upcoming month

I understand payments can be made to Piano Melodies Studio by:

I understand I will be charged a fee of $35.00 for any NSF cheques that I write

I understand any lessons canceled by my teacher will be credited to the next months invoice

I understand Piano Melodies Studio requires one month's notice should I wish to terminate lessons
